CONSENT - Please tell us what YOU understand when you have signed a DLA claim

When you signed your forms for Disability Living Allowance you gave consent to the following statement on the form.

 

 

"Consent

 

 

We may wish to contact your GP or persons or organisations involved with your treatment or care for information in relation to your award of benefit. This may include information in respect of your award.  You  do not have to agree to us contacting those persons or organisations.  If you do not, however, agree to us obtaining such information, it may mean that we are unable to obtain enough information to satisfy ourselves that you meet the conditions of entitlement for an award of benefit.

The Department for Work and Pensions or any doctor providing medical services on behalf of an organisation approved by the Secretary of State, may ask any person(s) or organisation(s) for any information, including medical information, which is needed to deal with your entitlement for benefit."
